VPAS forms

VPAS requires a number of forms to be completed when sending a baby for autopsy. 

The information provided assists the pathologist and scientists to manage the autopsy and transportation of the baby in line with the parent’s wishes and to address the clinical issues relevant to the death of the baby. 

Please also review the checklist to ensure all relevant results and information specified, such as antenatal imaging, pathology testing, and relevant medical and midwifery/nursing notes relating to the antenatal period and birth, are provided.
