The Women's Disability Liaison Officer is here to help individuals with disabilities, their families, and carers. They help you access the care and support you need during your hospital visit.

The Disability Liaison Officer (DLO) is available to assist with overcoming barriers related to disability and ensure inclusive, accessible care.

The DLO is part-time and offers regular support to current patients.

  • Work with you and your healthcare team to provide safer, more accessible care during your stay and specialist appointments.
  • Provide specific support for patients accessing maternity, gynaecological, sexual and reproductive health, allied health, and other services at the Women's.

Reasonable adjustments for your hospital visit

The Women’s is committed to providing accessible and inclusive care. Reasonable adjustments are changes made to ensure your visit is as comfortable as possible, addressing your specific needs.

What to bring to your hospital visit

  • A support person – A family member, friend, or carer to assist and communicate on your behalf.
  • Your care plan or medical information – Documents outlining your medical history, needs, and preferences.
  • List of needed adjustments – Specify any support you require, such as extra time or mobility assistance. You can also complete the Health Hub Disability Identifier Questionnaire.
  • Communication aids – If needed, bring a communication board, hearing aids, or a notepad.
  • Sensory tools – Items like noise-cancelling headphones or sunglasses for sensory sensitivities.
  • Comfort items – Familiar items like a favourite blanket or soothing music.
  • List of questions or concerns – Write down any questions about your care or adjustments.
  • Emergency contact information – A list of emergency contacts for extra support.
  • Medicines or treatment notes – A list of current medicines or treatment instructions.
  • Advance care directives – Documents like a living will or power of attorney.

How to inform your healthcare team about your disability and support needs

  1. Log in to your Health Hub app.
  2. Select ‘Menu,’ then scroll to ‘Questionnaires'.
  3. Choose 'Identification of Disability'.
  4. Complete the questionnaire and list needed supports.
  5. Submit the form to add it to your medical file.

This information helps your healthcare team provide the best care and support and will be kept confidential. Please update the questionnaire regularly to reflect any changes in your needs.

You can refer yourself or be referred by a health professional (with your permission). Contact the Disability Liaison Officer directly via email.

What to bring

  • Letters related to your disability diagnosis, support worker contact details, and your NDIS number
  • Your Medicare card
  • Your Health Care card (if you have one)
